Comprehensive Case Management Plan

Our Approach to Settlement

At Global Pathway, our settlement services are delivered through a structured, outcomes-driven model that aligns with and enforces the three core elements used by ECWA under the Community Support Program (CSP). This approach ensures clarity, accountability, and consistent support for entrants, applicants, and Australian supporters throughout the CSP process.

Our model supports refugees from arrival through long-term settlement, with clearly defined roles, responsibilities, and timeframes communicated to all parties.

1. Comprehensive Case Management Plan (CMP)

Within the first month of an entrant's arrival in Australia, Global Pathway develops a detailed and individualised Comprehensive Case Management Plan (CMP).

The CMP:

  • Is tailored to the entrant's specific needs and circumstances
  • Aligns with CSP settlement objectives and requirements
  • Is initiated at the initial settlement review
  • Is maintained as a live document, reviewed and updated monthly
  • Remains active for up to 12 months, concluding with a formal exit interview

Following exit, Global Pathway prepares a CMP summary and facilitates a structured handover to the organisation selected by the entrant for ongoing support. Entrants are fully informed of available options, and Global Pathway ensures the chosen organisation has the capacity and capability to support continued settlement.

A formal handover meeting is coordinated to ensure continuity of care and a seamless transition.

2. Outcomes-Based Settlement Support

Global Pathway delivers tailored settlement support identified within each CMP, ensuring entrants are supported to achieve effective and sustainable settlement outcomes under the CSP.

As part of this approach, Global Pathway coordinates a structured settlement education and orientation program, engaging relevant organisations and service providers to support entrants across key settlement areas, including:

Housing
Health and Wellbeing
Managing Money
Community Participation and Networking
Transport
Family Functioning and Social Support
Justice and Legal Awareness
Language Services
Education and Training
Employment

All services are delivered in a culturally responsive and multilingual manner, ensuring accessibility and understanding.

3. Final Interview, Referral and Ongoing Support

Towards the end of the initial 12-month settlement period, Global Pathway conducts a final settlement interview to assess whether CSP settlement outcomes have been achieved.

Where further support is required:

  • Appropriate referrals are made to relevant service providers
  • Entrants are reminded of their rights and responsibilities under the CSP
  • Clear guidance is provided regarding post-12-month support arrangements

A joint meeting is arranged between the entrant and the organisation that will manage settlement needs beyond the initial 12 months, potentially extending up to seven years. Global Pathway reassures entrants that assistance remains available beyond the initial settlement period if required.

Global Pathway provides comprehensive settlement services aligned with CSP requirements. There are no additional fees for these services.
